We apologize in advanced that we are often booked out a couple months at a time. You may need to check further dates to find our first availability. Please mark your calendar and set a reminder for your appointment after it has been scheduled.

We do require a $100.00 deposit for holding appointment time slots. This fee will go towards the final cost of your visit and is fully refundable if you need to cancel/reschedule your appointment before 48 hours of your appointment for any reason.
Because of our limited availability, if you miss your appointment, or do not reschedule/cancel before the 48 hour window, your $100 deposit will not be refunded.
Scheduling Instructions
- We highly recommend using a computer or laptop to schedule your appointment. Mobile devices such as phones or tablets can be glitchy and cause issues.
- After inputting your appointment information, to save your time slot please click “Confirm”. You should receive an email to confirm your appointment.
- Follow your email instructions to create your Charm Portal account. This is time sensitive and should be created within 2 days, or you may be timed-out.
- After your Charm account is created, log in to your Charm account to complete the online intake forms. The intake forms can take +30 minutes to complete. Please complete your intake forms as soon as possible. This is best done on a computer, not on a mobile device.
- If your forms are not done by at least 48 hours before your appointment, we will not have consent for treatment and cannot see you, which will require rescheduling the appointment to a later date.
